Submit your records and intake information

Provide the records relevant to your claimed condition. Depending on the case, these may include service treatment records, military personnel records, VA and private medical records, prior C&P examination findings, VA decision letters, and other supporting evidence. We use this information to understand the claimed condition and the medical question that needs to be addressed.

A qualified medical provider reviews the available records, medical history, relevant clinical information, and the claimed relationship to military service or an existing service-connected condition. We evaluate whether the available evidence provides an appropriate medical basis for a nexus opinion.

If we do not believe the evidence supports a medically defensible opinion, we will tell you rather than provide a letter simply because one was requested.

When the evidence supports an opinion, the provider prepares an individualized nexus letter explaining the records reviewed, relevant medical findings, clinical reasoning, and medical conclusion. When appropriate, peer-reviewed research and other authoritative medical sources may be incorporated to support the rationale.

The completed letter is signed by the evaluating provider and delivered to you for use as supporting medical evidence in your VA disability claim.
